The easiest place to report is through ALA:

Reporting is essential for protecting the Constitution, intellectual freedom, and the right to read. ALA and ULA have expertise and resources to help librarians through challenges small and large. Too often we only discuss challenges that become media circuses. Truthfully though, challenges happen everyday in the state of Utah formally and informally.
